Please arrive 10-15 minutes before your scheduled appointment time. If you have paperwork to complete and are unable to access through the online patient portal, please arrive 15-30 minutes before your scheduled appointment time to complete in the office. If you need to reschedule, please text or call us no less than 24 hours before your scheduled appointment time to avoid any late cancellation fees, see below.
Late Cancellation Fees: 50% of the service cost booked or $100 for medical consults/tox appointments or $25 for skincare consults if our office is notified less than 24 hours in the event you need to cancel or reschedule for any reason. Clients more than 15 minutes late for their appointment will need to be rescheduled and will be subject to late cancellation fees.
No-Show Fees: 100% of the service cost booked or $150 for medical consults/tox appointments or $50 for skincare consults if our office has no prior notice.
Skincare consultations cancelled 3 times will require a $50 deposit. Medical consultations cancelled/rescheduled 3 times will forfeit all consultation deposits regardless of notice. Any medical appointments rescheduled or cancelled in an excess of 3 times may result in a required deposit in order to reschedule any medical appointments. Further cancellations may result in patient dismissal from the practice at the discretion of the provider.
The credit card on file will be charged ONLY if these policies are broken. Thank you for your understanding.
