Please fill out the form below to REQUEST an appointment. Please note that your appointment is NOT confirmed until you receive a confirmation email. We try to approve all requests within 48 hours except on weekends. If you receive an email stating that there was a CONFLICT with your request, that means that someone requested the appointment prior to your request. Thus, you will need to go online to request another time and date. We understand that things come up, but we respectfully ask that any cancellations or rescheduling be made at least 48 hours before your scheduled appointment. Please note that we adhere to a strict cancellation/no-show policy. We require at least 48 hours notice prior to a cancellation/reschedule, or the client will incur a $100 "late cancellation fee". If there is a deposit being held for this appointment, the client will lose their deposit. This policy is in place to compensate for the time commitment on the part of the practice.
