Please be sure to review our Cancellation Policy on our website, which requires over 24 hour notice of changes to avoid forfeit of your deposit.

IF YOU ARE BOOKING FOR WEIGHT LOSS: you must bring in a copy of labs dated within two years that include a CBC, CMP, lipid panel, TSH, and fasting blood sugar to your appointment. If you do not have that available we can give you a lab slip at your first appointment.
